Sentinel Node Biopsie beim Mammakarzinom. Technik und Indikation – Standard of Care. Übersichtsarbeit

Die Wächterlymphknotenbiopsie beim Mammakarzinom erwies sich als ein sehr sicheres diagnostisches Konzept zur Bestimmung des Lymphknotenstatus. Mit der kombinierten Anwendung von Radiokolloiden und Blaufarbstoffen sowohl oberflächlich (intra- oder subkutan über dem Tumor, sub- oder periareolär) als auch tief (peritumoral, intra- oder subtumoral) lassen sich nach Erlernung der Methode Auffindungsraten von > 95% und niedrige falsch-negativ-Raten von < 5% erzielen. Allgemein anerkannte Indikationen stellen uni- und multifokale Tumore bis 3 cm und ein klinisch negativer axillärer Lymphknotenstatus dar. Bei bestimmten Formen eines Ductalen-Carcinoma-in-situ als auch nach präoperativer Chemotherapie erwies sich die Sentinel Node Biopsie als sinnvoll und sicher, umstritten ist sie derzeit noch bei multizentrischen Mammakarzinomen und bei extraaxillären Lymphknoten. Voraussetzung zur Erfüllung der international vorgegebenen hohen Erwartungen sind eine entsprechende interdisziplinär abgesprochne Struktur-, Prozess- und Ergebnisqualität sowie definierte Qualitätsindikatoren

mit laufender Dokumentation. Die Teilnahme an nationalen Studien wäre empfehlenswert.

Sentinel Node Biopsy in Breast Cancer. Technique and Indication – Standard of Care

Sentinel node biopsy proved to be a useful and accurate procedure for axillary staging in breast cancer. Combination of the current techniques with radiocolloids and blue dye, applicated superficial (intradermal, subdermal, peri- and subareolar) and deep (peritumoral, intratumoral, subtumoral) enable high indentification rates und negative predictive values. It should be performed by surgeons, pathologists and management teams of nuclear medicine with appropriate training and experience. Accepted indications are uni- and multifical tumor less than 3 cm without suspicious findings in the axilla, further in patients with large ductal carcinoma in situ and after preoperative chemotherapy. Sentinel node biopsy is still under discussion in patients with multicentric tumors and nodes outside of the axilla, although there are good results in applying this method, few data exist.

Sentinel node biopsy is a team approach, prerequisites are good cooperation and well defined stuctures of qualities and documentation. Participation in national clinical studies are recommended.
