Bipolar Affective Disorder and Suicidality

Summary
Suicide behavior is prevalent in individuals with BAP (bipolar affective disorder). The suicide rate in BAP individuals is approximately 20fold greater than that of the general population. Indicators for suicide attempts and completed suicide are history of suicide attempts, percentage of time spent in depressed state, hostility and sexual and physical abuse in childhoods. Adjuvant somatic illness (obesity, metabolic syndrome) can enhance suicidality. We have no convincing biological markers of suicidal behavior; the levels of monoamine metabolites in cerebrospinal fluid are studied. Compelling evidence supports an antisuicide effect has only long term lithium treatment. In opposite unimodal antidepressants may engender or exacerbation of suicidality in susceptible individuals with BAP. Results of recent studies suggest that adjunctive psychosocial interventions may further reduce suicide risk in bipolar individuals.
