Leg Panniculitis as the First Manifestation of Crohn’s Disease

Summary
Authors refer a case of a 26-year-old, otherwise healthy, man with panniculitis of lower extremities recurring for one year. Skin biopsy finding showed non-caseating granulomatous inflammation. Because of cutaneous findings and ASCA (Anti-Saccharomyces Cerevisiae Antibodies) positivity colonoscopy was performed with diagnosis of Crohn’s disease. Treatment with prednisone, mesalazine and azathioprine had no effect. Biological treatment with infliximab resulted in remission of both cutaneous symptoms and gastrointestinal involvement. Authors review current knowledge on metastatic Crohn’s disease.
