Rare case of benign prostatic hyperplasia with extensive fibrolipomatous metaplasia of the prostatic stroma

Summary

We report a case of a 79-year-old patient with benign prostatic hyperplasia. He was admitted to the department of urology at our hospital to undergo a transurethral resection of the prostate.

Pathological examination confirmed benign prostatic hyperplasia with areas of extensive fibrolipomatous stromal metaplasia. While the presence of adipose tissue in the prostatic stroma is sporadic, fibrolipomatous stromal metaplasia is an extremely rare finding. It has been described in the literature only once. The main diagnostic task in finding adipose tissue in the prostate stroma is to determine whether it is really benign prostatic tissue with lipomatous stromal rearrangement or the spread of tumor cells of well-differentiated acinar prostatic adenocarcinoma into periprostatic adipose tissue.
