Indications, timing and initiation of mobile specialized palliative care and the role of the general practitioner

Summary

More than three quarters of the population of the Czech Republic wish to die at home. In fact, most of deceased people die in medical facilities. Terminal hospitalizations may occur due to complex symptoms requiring frequent medical interventions that cannot be provided in the routine of visiting care by a general practitioner. Care for terminally ill patients with severe symptoms in their homes is provided by mobile hospices, i.e. mobile specialized palliative care (MSPC) teams. Therefore, the development and expansion of MSPP appears to be a very relevant topic. The article is mainly focused on the indication, timing and initiation of the MSPC. GPs have a key role in these processes in a number of ways. For various reasons, MSPC is often involved late, when patients benefit only partially from it and their quality of life is significantly deteriorated. The case studies point out the possible differences in what can be offered to a patient at the end of life during a short and a longer period use of MSPC services.
