Incidental discovery of renal carcinoma based on liver visualization during Tc-99m MAA lung scan

Summary
A 73-year-old man with a history of diabetes and hypertension was admitted with a 1-day history of painful swelling of the left leg, but no dyspnea. He was referred for radionuclide venography/lung scan to exclude deep venous thrombosis. On the pulmonary images the liver could be clearly seen but no other organ was identified. This raised the suspicion for a cavoportal shunt. Renal carcinoma with extension into the right renal vein and the IVC superior to the renal vein was then confirmed on CT scan. The patient died before surgery could be performed.
