Modification of reconstruction parameters of IQ-SPECT study to maintain continuity of left ventricle functional parameters processing results

Summary

Introduction:
IQ-SPECT system represents the technological effort in myocardial perfusion imaging. However, we observed substantial discrepancies in acquired perfusion and functional parameters of the left ventricle when comparing those from IQ-SPECT and conventional tomographic camera. We tried to find the optimisation of the IQ-SPECT study reconstruction to get minimal differences between the systems concerning the functional parameters of the left ventricle.

Subjects and methods:
The study group consists of 42 patients (27 male, 15 female, mean age 62.3 ± 9.2 years) who underwent gated-SPECT study at rest condition on both systems. The IQ-SPECT data were processed with Flash3D iterative reconstruction algorithm; various settings of Butterworth filter were serially applied. Reconstructed studies were assessed in Corridor4DM application. Values of ejection fraction (EF), end-diastolic and end-systolic volumes (EDV, ESV) of the left ventricle obtained from conventional system were compared with those from initial and filtered IQ-SPECT studies.

Results:
The EF values were systematically higher on conventional system, EDV values were systematically lower, ESV values showed proportional error for the initial setting of IQ-SPECT reconstruction. Lowest absolute differences for EF values were observed for the Butterworth filtering of order 1 and cut-off 0.32. Comparing outcomes from conventional system and such optimised IQ-SPECT study, EF, EDV and ESV showed neither systematic nor proportional error.

Conclusion:
When adopting new method, the reproducibility of the outcomes during the follow-up is essential. With postprocessing of IQ-SPECT study there is possibility to adjust the obtained functional parameters of the left ventricle to achieve minimal and clinically irrelevant differences to conventional tomographic method.
