Biologic behavior of adenoid cystic carcinoma of major salivary glands

Summary

Introduction: Adenoid cystic carcinoma (AdCC) is a slowly growing tumor for which recurrences and distant metastases are common. It is one of the most frequent salivary malignancies and accounts for 5% of these tumours. Most of clinical studies collect minor and major salivary gland tumours, but just a few focus on the latter.

Methods: In our retrospective study we investigate biological behaviour and survival of 32 patients with AdCC of major salivary gland treated on 3 clinics – University hospital Olomouc and University hospital Plzeň between 1989–2014.

Results: The 5-year, 10-year and 15-year overall survival (OS) in the cohort were 79.8%, 75.8% and 57.8% respectively. For low stage tumours (stage I + II) OS were at 5,10 and 15 years the same – 87.5%, while for stage III + IV were 70.7%, 62.9% and 31.4%, respectively. Corresponding disease specific survival (DSS) and disease free survival (DFS) in local limited disease has reached 96.0%, 91.2% and 69.5%, and 79.0%, 69.0% and 30.7%. During the same time interval we achieved local control in 85.9%, 75.4% and 60.3%, respectively, while distant metastases incidence increased from 10.4% to 24.4% and 48.2%.

Conclusion: Our results confirm difficult control of advanced stages of AdCC and are consistent with previous studies. Therefore lifelong follow-up is advised.
