Bilateral, traumatic rupture of the quadriceps tendon – a physiotherapeutic pitfall and case report

Summary

Bilateral, simultaneous rupture of the quadriceps tendon is very rare. It is serious damage of the extension apparatus of the knee joint that requires prompt diagnosis and surgical repair. Predisposition to this injury may be caused by degenerative changes due to another disease. It may be associated with diabetes mellitus, obesity, gout, chronic renal failure or chronic use of steroids. Clinical findings typically include the triad of acute pain, inability to extend the knee actively and suprapatellar gap. Complete ruptures are best treated with early surgical repair. In our opinion the quality of postoperative physiotherapy contributes considerably to the final outcome. The main goal of this paper is to point out the rare traumatic injuries of the extensor apparatus of both knees with an overview of relevant information from the available literature and the presentation of the methodology of kinesiotherapy after this type of injury. The case report is with the diagnosis of bilateral quadriceps tendon rupture. He was treated surgically. The tendons were refixed. He walked early with a symmetrical load on his lower limbs. Both legs were immobilised for 5 weeks postoperatively. He completed inpatient rehabilitation and then returned to his pre-traumatic functional level.
