Sexual function sparing radical cystectomy

Summary

The aim of this work is to review all published papers regarding sexual functions sparing cystectomy.

We reviewed all articles in English language identified in Medline database, under key words: sexual, potency, erection, ejaculation, ejaculatory, sparing and cystectomy published until 2007. All data were fully analysed.

The potency is preserved in 17–100 % of patients who underwent nerve sparing radical cystectomy. The preservation of potency is decreasing in patients in age group of 60–65 years and more and in those who underwent urethrectomy or unilateral procedure. The daily continence is preserved in 80–100 %, night continence in 47–75 % of patients. The potency rate of 80–100 % is observed in those patients, where vas deferens, seminal vesicles and part of the prostate were preserved. There is 79–100 % rate of day continence and 31–100 % rate of night continence. In those cases where cord, seminal vesicles and whole prostate were preserved the potency rate is 70–100 % and antegrade ejaculation is observed in 50 % of patients. In these patients daily continence is 95–100 %, and night continence is 74–88 %. Furthermore, we focused on other relevant details in our paper: number and characteristics of patients, period of recruitment, oncological indications and results, surgical methods of nerve sparing cystectomy, operating times and follow-up of the patients.

It is clear from the published data, that the surgical techniques have made a great progress in the last ten years, and there is a big chance of preservation of sexual functions after radical cystectomy in male patients. Mostly younger patients with favourable oncological characteristics of the tumour could benefit from this kind of surgery. More precise indication criteria will have to be defined.
