Role of rs17817449 FTO gene polymorphism in patients with type 1. diabetes in terms of obesity and diabetes control

Summary

Objective: On a Czech population of type 1 diabetics, we investigated the influence of the risk G-allele of the rs17817449 FTO gene polymorphism on obesity and diabetes control.

Design: Retrospective study.

Settings: Diabetes Centre, Institute for Clinical and Experimental Medicine, Vídeňská 1958/9, 140 21 Prague 4, Czech Republic.

Material and methods: Fulfilling strict inclusion criteria a cohort of 171 men and 115 premenopausal women with type 1 diabetes were selected (C peptide level under 0.2 nmol/L), further subdivided into groups according to the genotype on TT vs. GG homozygotes for the rs17817449 FTO gene polymorphism. In a stable state of the diabetes disease, measurements were collected: weight, height, body mass index (BMI), insulin requirements (IU/kg), mean glycosylated haemoglobin per year (HbA1c). A statistical analysis was performed using the program GraphPad Prism 5.0. California.

Results: The risk G-allele in men as well as premenopausal women with type 1 diabetes is not associated with obesity and does not explain the difference in BMI and HbA1c (GG/TT men: BMI 26.8/25.8 kg/m2, HbA1c 62.8/62.7 mmol/mol; women: BMI 24.1/24.6 kg/m2, HbA1c 64.3/63.1 mmol/mol). In a gender-specific manner the GG homozygous men were treated with statistically significantly higher insulin doses in comparison to TT homozygotes (0.63 vs. 0.57 IU/kg).

Conclusion: We have not found a significant influence of the FTO gene polymorphism on obesity risk or worse diabetes control in type 1 diabetics. But we found a connection between higher insulin doses in male GG homozygotes, which may attribute the relationship of the FTO gene polymorphisms to induction of insulin resistance.
