Transpupilární termoterapieneovaskulárních membrán meziostatními léčebnými postupy

Choroidální naovaskularizace (CNV) při věkem podmíněné makulární degeneraci je jednou z vedoucích příčin poklesu zrakové ostrosti u dospělých. Přestože u většiny pacientů se rozvíjí okultní formy nemoci, dosavadní terapeutické snahy byly zaměřeny na menší procento očí s přesně ohraničenými klasickými formami membrán. Transpupilární termoterapie je novou, nadějnou metodou v terapii okultních forem CNV.V naší práci demonstrujeme efektivnost této terapie. 50 očí s CNV při věkem podmíněné makulární degeneraci bylo podrobeno transpupilární termoterapii a sledováno v retrospektivní, selektivní studii. Používali jsme diodový laser (810 nm), velikost stopy byla mezi 0,8 až 3,0 mm a energie se pohybovala mezi 700-800 mW. Čas pro jednu stopu byl stanoven na 60 sekund.Transpupilární termoterapie je velmi dobrou metodou terapie CNV. Ve velkém procentu případů dochází k uzavření membrány, přičemž zrakové funkce zůstávají stabilizovány, nebo se dokonce zlepšují.

Choroidal neovascularization (CNV) secondary to age-related macular degenera-tion is a leading cause of vision loss in adults. Although most patients present with occult CNV, treatment has focused on the small percentage of eyes with well-deli-neated, classic CNV. Transpupillary thermotherapy is a recent advancement in the management of occult CNV. We demonstr ated the effectiveness of transpupillary thermotherapy (TTT) for the treatment of occult choroidal neovascularization (CNV). In a retrospective, case selected trial 50 eyes with CNV secondary to age related macular degeneration (AMD) were studied. The eyes with CNV were treated with diode laser (810 nm) TTT. Laser beam sizes ranged between 0.8 and 3.0 mm and power settings between 700-800 mW. Treatment was given in one area for 1 minute.Transpupillary thermotherapy is a potential treatment for CNV. It is able to close choroidal neovascularization while maintaining visual function in patients with classic and especially occult disease.
