Impact of Inhalation of Factory-Prepared Equimolar Mixture Nitrous Oxide/Oxygen during Dental Setting on Children´s Behavior

Summary
Introduction and aim:
The paper focuses on the use of a factory-prepared equimolar mixture of nitrous oxide and oxygen (50 % N2O / 50 % O2) during dental treatment of children. Twelve-month results of using this method at the Department of Paediatric Dentistry of ICEDM are being presented.
Methods:
The authors describe the technique of the administration of 50 % N2O / 50 % O2 using inhalation through a demand valve and a mouth piece or face mask. The use of the mixture in the outpatient dental setting is demonstrated on paediatric patients treated at the Department of Paediatric Dentistry in the period from June 2012 to June 2013. The effect of a conscious inhalation sedation using 50 % N2O / 50 % O2 on behaviour of the treated patients is being discussed. For the assessment of the children´s behaviour, a modified four-degree Frankl scale of behavior is being used.
Results:
A group of 86 children aged 3 to 12years was treated at department over the period from June 2012 to June 2013. We recorded the amount and character of the procedures performed under the conscious inhalation sedation, such as fillings and/or extractions (of deciduous or permanent teeth) or the need for dental surgery. Using the modified Frankl scale we graded the children´s behaviour during dental visits in the past, during the first visit at our department and during the follow-up. The presence or absence of amnesia after the treatment was recorded both in new and long-term patients of the department. Chi-square test at the 5% level of significance was used to test relationships between the variables. Statistical analysis revealed positive behavioural changes in the paediatric patients in association with the treatment under the conscious inhalation sedation using 50 % N2O / 50 % O2.
Conclusion:
The use of 50 % N2O / 50 % O2 has a positive impact on the behaviour of children during dental treatment independent on the dental procedure they undergo. Repeated treatment using 50 % N2O / 50 % O2 is associated with a more pronounced behavioral changes than a single treatment. Induction of amnesia after the treatment, which is more frequent in treatment- naive children and in children, that request long-term dental treatment at the department, is significant.
