Akutní infarkt myokardu v graviditě je raritním onemocněním, nicméně v posledních letechfrekvence jeho výskytu na podkladě více faktorů stoupá.V patogenezi nestojí jednoznačně na prvním místě koronární ateroskleróza, uplatňuje se zdeřada dalších mechanismů, jedním z nich je i hyperkoagulační stav v graviditě.Z hlediska léčby je na prvním místě rychlé dosažení reperfuze ischemického myokardu otevře-ním infarktové tepny, čehož je možné dosáhnout jednak užitím fibrinolytické léčby a dnes již jena vybraných pracovištích první metodou volby primární koronární angioplastika. Užití tétometody jako bezpečné a efektivní léčby dokumentuje i naše kazuistika 34leté nemocné vedruhém trimestru gravidity.

Overview

Acute myocardial infarction during pregnancy is a rare disease, nevertheless in recent yearsits incidence is due to several factors increasing.In the pathogenesis the first place is not held unequivocally by coronary atherosclerosis. Alsosome other mechanisms are involved, one of them being the hypercoagulation state duringpregnancy.From the therapeutic aspect the first place is held by achievement of reperfusion of theischaemic heart muscle by opening of the infarcted artery which can be achieved by fibrinolytictreatment and nowadays by the method of first choice - coronary angioplasty - which can beperformed in somer selected departments. The application of this method as safe and effectivetreatment is apparent also from the case-history of a 34 year old patient in the second trimesterof pregnancy which is presented by the authors.
