Therapeutic strategies to optimize bone metabolism in anorexia nervosa

Summary

The occurrence of anorexia nervosa at the critical time when peak bone mass (PBM) is accrued, significantly interferes with bone metabolism and represents lifelong burden and risk for developing of premature osteoporosis. Nutritional refeeding, dietary intake allowance for calcium and vitamin D, together with spontaneous restoration of menstrual function is the safest and the most effective strategy for improvement of bone mineral density. Physical activity should be regulated according to the stage of recovery. „Physiological“ estrogen replacement increases bone mineral density, but does not reach the "catch-up". Potential therapy strategies (recombinant IGF-1, bisphosphonates) have been investigated. Long-term persistence of the disease and frequent relapses delay the recovery, as well as the normalization of bone metabolism.
