Effects of Insulin in High-Renin and Low-Renin Hypertension
PŮSOBENÍ INZULINU U VYSOKORENINOVÝCH ANÍZKORENINOVÝCH HYPERTENZÍ
Východisko.
Zhoršená inzulinová rezistence byla popsána u nemocných s esenciální hypertenzí. Cílem naší studiebylo ověřit na receptorové a postreceptorové úrovni přítomnost inzulinové rezistence u hypertenze s vysokoua nízkou reninovou aktivitou.Metody a výsledky. Vyšetřeno bylo šest pacientů se stenózou renální arterie (průměrný věk 51 ± 12 let, rozmezí20–65 let, BMI: 27,0 ± 2,0 kg.m-2) a šest pacientů s primárním hyperaldosteronizmem (průměrný věk 50 ± 13 let,rozmezí 28–61 let, BMI: 26,9 ± 3,2 kg.m-2). Diagnóza byla stanovena pomocí laboratorních vyšetření, výpočetnítomografie a duplexní sonografie. Krevní tlak byl monitorován po dobu 24 hodin Spacelab tonometrem (STk: 161± 29 mmHg, DTk: 98 ± 12 mmHg vs. STk: 168 ± 21 mmHg, DTk: 103 ± 9 mmHg, plazmatická reninová aktivitabyla 8,1 ± 5,6 vs. 0,3 ± 0,4 ng.ml-1h-1, p

Background.
Setback in insulin resistance has been described in patients with essential hypertension. The aim ofour study was to verify at the receptor and postreceptor levels the presence of insulin resistance in patients with highor low renin activity.Methods and Results. Six patients with the renal artery stenosis (20 to 65 years, average age was 51 ± 12 years,BMI: 27 ± 2.0 kg.m2) and six patients with primary hyperaldosteronism (28 to 61 years, average age was 50 ± 13years, BMI: 26.9 ± 3.2 kg.m2) were investigated. Their diagnose was confirmed by laboratory examination, computertomography, and duplex sonography.
