Aspiration Cytology Versus Perioperative Histology in the Diagnosis of Thyroid Nodules

Summary
Aim:
Fine-needle aspiration cytology (FNAC) plays an important role in the differential diagnosis of thyroid nodules. On the other hand, perioperative histology (POH) is not a standard method in the diagnosis of nodular goiter.
Material and methods:
The authors retrospectively analyzed patients who were surgically treated for thyroid disease at the Department of Otorhinolaryngology, Head and Neck Surgery, Comenius University, Jessenius Faculty of Medicine, University Hospital in Martin, Slovakia from the 1st of January 2005 to the 31st of December 2014 for thyroid disease. The, indication and accuracy of FNAC and POH in the differential diagnosis of thyroid nodules were evaluated.
Results:
A total of 2117 patients were surgically treated for thyroid disease at the Department of ENT, Head and Neck Surgery, CU JMF, UH in Martin, Slovakia between 2005 and 2014. The most frequent indication for FNAC and POH was solitary thyroid nodule. The size of the thyroid nodule did not significantly affect the accuracy of FNAC or POH (P=0.107, P=0.857, respectively). High sensitivity and specificity in the diagnosis of malignant thyroid nodule was observed by using aspiration cytology (78 %, 91 %, respectively) and intraoperative histopathological examination (95 %, 100 %, respectively).
Conclusion:
Aspiration cytology and perioperative histopathology have high sensitivity and specificity in the diagnosis of malignant thyroid tumor. The accuracy of aspiration cytology and perioperative histology depends mainly on the experience of investigating cytopathologist and they are not affected by the size of the nodule.
