Acute Achilles tendon rupture

Summary
Aim of the study:
The author evaluates the results of surgical treatment of acute closed rupture of the Achilles tendon at his workplace and performs a comparison between different surgical techniques.
Material and Methods:
The author retrospectively assessed findings of a group of 54 patients presented with acute simplex Achilles tendon rupture by using of Leppilathi score in 2008–2012. 16 patients were treated with an invasive technique, 25 patients with semiinvasive technique and 13 patients with percutaneous technique. Orthesis VacoPed (earlier VacoAchill) with the possibility of immediate functional therapy was applied in all cases.
Results:
All cases of Achilles tendon rupture healed completely. There was no difference in the functional outcome in total among all the used techniques. Healing complications of the wound in the form of superficial hemorrhage occurred in the total amount of 12 patients (22.2 %). Deep wound infection associated with secondary healing was observed twice (3.7 %). The main difference was found during wound healing after open surgery method, when the author noted 37.5 % of complications compared with 15.4 % with the percutaneous method. In total, nine cases of phlebothrombosis (16.7 %) and five cases of irritation of nervus suralis (9.3 %) were reported. No relapse of Achilles tendon rupture was reported.
Conclusion:
Percutaneous technique combined with immediate functional (load – movement) therapy has been manifested as the most proper method of treatment that enables immediate rehabilitation and patient’s integrations into the social life.
