Treatment of the radial head fractures by endoprothesis

Summary
Objective of the study:
This study summarizes and compares modern methods of treatment for comminuted radial head fractures - prosthetic replacement, open reduction internal fixation (ORIF) of the radial head with the obsolete radial head resection.
Material and methods:
This is a retrospective comparative study evaluationg 37 patients with Mason type III radial head fractures treated at our department in the period from January 1st 2007 to December 31, 2013. This group is compared to five patients treated treated between 2004 and 2006 by radial head resection.
Results:
The average Mayo Elbow Performance Score (MEPS) was after prosthetic replacement in the excellent range, while after ORIF the results were found to be “only” good. The results of radial head resection were only satisfactory. During prosthetis replacement oft he radial head we observed the least complications.
Conclusion:
According to our experiences treatmnet of Mason III-type radial head fractures by prosthetic replacement is an excellent method, combined with surgical management of accompanying elbow injuries. It is fully indicated in all cases, where reconstruction is very complicated or even impossible.
