The importance of antimicrobial prophylaxis on urgent appendectomy

Summary

Introduction:
Prophylactic antibiotics administration before surgery evidently decreases surgical site infections incidence. Antimicrobial prophylaxis guideline was installed on author´s workplace in 2011. In 2012 we demanded consistently compliance of this guideline especially given to time interval of administration in the same time with premedication ordered by anesthetist, it means 30–60 minutes before surgery.

Target:
Evaluation of surgical site infections incidence after appendectomy as model surgical procedure in dependence to antimicrobial prophylaxis.

Method:
Retrospective analysis of file of 134 patients operated in 24 hours after admission to hospital for suspected acute appendicitis and comparison results with previous years.

Results:
Consistent compliance to antimicrobial prophylaxis guideline including time interval of admission in 94,8 % of patients led to significant decrease of surgical site infections from 7,74 % in 2011 to 0,7 % in 2012.

Conclusion:
Consistent compliance to antimicrobial prophylaxis guideline led in model surgery procedure to significant decrease of surgical site infections incidence. As the most important factor of efficiency appears the time interval of admission 30–60 minutes before surgery starts.
