Nejrizikovější faktory pro vznik pankreatitidy po ERCP – systematický přehled zaměřený na prevenci

Súhrn

Pankreatitida po endoskopické retrográdní cholangiopankreatografii (ERCP) je nejčastější a nejnebezpečnější komplikací diagnostické a terapeutické ERCP. Pro identifikaci rizikových pacientů a následný výběr vhodného alternativního způsobu diagnostiky je potřeba provést multifaktoriální studii rizikových faktorů vzniku PEP. Taková studie byla provedena. Dva nezávislí výzkumníci (MT, SH R) hledali relevantní studie publikované mezi 1/1/2017 a 1/10/2022. Literaturu v anglickém jazyce prohledali pomocí MEDLINE přes PubMed, EMBASETM přes Ovid, the Cochrane Library a Trip. V jiných jazycích hledali přes Magiran, SID, KoreaMed a LILACS. Hledaný termín byl endoskopická retrográdní cholangiopankreatografie NEBO pankreatitida NEBO rizikové factory NEBO ERCP NEBO systematický přehled. Vybráno bylo 174 studií. Pět z nich splňovalo požadovaná kritéria a byly definitivně vybrány. Tento článek prezentuje současné znalosti o nejrizikovějších faktorech pro vznik post--ERCP pankreatitidy, která je nejčastější komplikací ERCP. Panuje debata o patofyziologii PEP. Díky její komplexitě by prevence PEP měla zahrnovat úvahu o různých predisponujících faktorech, jako rizikovost pacientů, farmakologická profylaxe a metodou provedení. Věk, pohlaví a předchozí pozitivní anamnéza nemoci patří mezi nejrizikovější faktory pro vznik pankreatitidy po ERCP.
