Summary

Design:

The study was aimed at determining the frequency of sensitisation to the European Standard Series allergens and 31 selected adjuvants of pharmaceutical, cosmetic, and industrial preparations in patients with chronic hand dermatitis examined during the years 2001–2016.

Patients and methods:

The cohort consisted of 1969 patients (749 men, 1220 women, average age 41.6 years), 951 of them (310 men, 641 women) were patients with basic diagnosis of atopic eczema. Contact allergens of Chemotechnique company and Curatest strips of Lohmann&Rauscher company were used for the testing. The period of patch test application was 48 hours; reactions were evaluated after 48, 72, 96 hours according to ICDRG rules.

Results:

The highest frequency of sensitisation to the European standard series allergens was detected for nickel sulfate 15.5%, cobalt chloride 5.5%, balsam of Peru 5.1%, Kathon CG 4.6%, fragrance-mix I 4.2%, colophony and propolis 4%, potassium dichromate, and formaldehyde 3.7%. In adjuvants the highest sensitisation frequency was to Kathon CG 4.6%, formaldehyde 3.7%, benzalkonium chloride, and alcoholes adipis lanae 1.9%, bronopol 1.5%, dodecyl gallate, and 1,3,5-tris(2-hydroxyethyl)-hexahydrotriazine 1.4%, chloracetamide, Bioban CS-1135, and methylene-bis(methyloxazolidine) 1.3%, Quaternium-15, 1,2-dibromo-2,4-dicyanobutane/phenoxyethanol, Amerchol L101, and diethanolamine 1.2%. Sensitisation to at least 1 allergen was found in 973 patients, i.e. 68.9% of the cohort (atopics 28.6%, non-atopics 68.9%).

Conclusion:

Altogether 1941 allergic reactions were detected with the clinical relevance of 58.7%. After performing patch tests the distribution of the cohort was as follows: allergic contact dermatitis 684 patients, atopic dermatitis 679, irritant contact dermatitis 317, atopic and allergic contact dermatitis 289. In 251 patients (of whom 56 had basic diagnosis of atopic dermatitis), i.e. 12.7% of the cohort, sensitisation was connected with their occupation; the cause being most often auxiliary substances of industrial fluids.
