Urine survivin in nonivasive diagnosis of transitional cell carcinoma of the urinary bladder

Summary
Aim:
To assess performance of survivin level in urine for noninvasive detection of urinary bladder cancer.
Materials and methods:
There were 151 patients enrolled in this study (84 patients with urinary bladder tumoru and 67 screenings) from July 2004 to January 2006. Patiens were divided into different groups according to tumour stage, grade, multiplicity, size, new cases and recidives.
Results:
The mean survivin level in patients with tumour was 32.4 pg/ml, and 22.4 pg/ml in the screenings (p = 0.001). The mean survivin level of patients depending tumour stage, grade and size was significant. Cut-off of urine survivin level was estimated at 33.9 pg/ml with specificity 90%, sensitivity 45%, positive and negative predictive value 45%, 86% and 53% respectively. Sensitivity was highest in the group of patients with high grade tumours (68–100%), in large tumours (80%) and in tumours larger than 5 cm (79%).
Conclusions:
Our results demonstrate that high level of survivin in urine corelates with the tumour stage and size as well as with the tumour grade. Survivin level in urine can be used as predictor of high stage and low diferenciation of bladder tumour.
