Summary
The Hospital pharmacy has been part of IKEM since 1994. It provides complex full pharmaceutical care for both, hospital wards of IKEM and for the public. Besides medication dispensing service patients can use a pharmacy consulting service which is implemented according to the guidelines of the Czech Chamber of Pharmacists. Main purpose of the consultating service is to contribute to rationalization of drug therapy and medication safety. Furthermore it also offers a great possibility for monitoring of disease risk factors and their management.
Most drug related problems are preventable. One of the ways is to involve a pharmacist in their identifing, solving and preventing. Among other specialized activities of hospital pharmacy belongs providing pharmaceutical care on wards, medicine information center and therapeutic drug monitoring.
Preparation of individualized parental nutrition is integral part of patient care in IKEM.
Efficient interdisciplinary cooperation between doctors and pharmacists is an essential point for clinical based activities in the pharmacy.
