Indapamide induced hyponatremia in an 83 year old patient

Summary

Hyponatremia is one of the most common electrolyte disorders in old age. As age advances the occurrence of hyponatremia risk factors increases. Thiazide and thiazide-like diuretics and antidepressants belong to drug classes with the highest incidence of adverse drug reaction – hyponatremia. In the case study we present an 83 years old female with indapamide-induced severe symptomatic hyponatremia with hypoosmolar hypovolemia which led to hospitalization. In thorough diagnostic process we were able to identify several other factors predisposing to hyponatremia while on diuretics – concomitant treatment with SSRI, acute infection, diabetes mellitus, chronic malnutrition. Treatment of hyponatremia – mainly stopping hyponatremia-inducing drugs, re-hydration with isoosmolar fluids, correction of hypomagnesemia, antibiotic treatment of infection led to clinical status improvement and increase of S-Na values. However, even after 4 weeks of treatment serum – Na concentrations did not return to the normal range.
