Summary

Hypoalbuminemia is a frequent finding in premature newborn infants. Use of albumin is not always recommended for treatment of edemas in preterm neonates. When albumin treatment is being considered, it is necessary to take into account capillary permeability condition and lymphatic system function. Albumin substitution may prevent developing edemas only if there are albumin losses and, at the same time, normal capillary permeability and function of lymphatic system.

The authors present a rare case of subdural fluid collection in a premature newborn infant. The ultrasound finding reminding brain atrophy had been a result of a serious hypoalbuminemia caused by increased albumin loss by means of pleural drains. „Empiric“ supplementation of albumin was found to be a successful method of treatment in the case reported.
