Colorectal cancer screening in the geriatric population

Summary

Colorectal cancer (CRC) is one of the most common malignant diseases in the world and Czech population. Screening programme for CRC was initiated in 2000. It has no upper age limit and it is entirely up to the referring physician to decide whether the screening is beneficial for the patient. Our paper presents updated data and evidence to support the decision on the indication of CRC screening in the geriatric population. The greatest benefit to the patient comes from early screening, with colonoscopy, which can detect endoscopically safely removable precancerous lesions. Careful consideration of the indication for screening is particularly relevant to the population over 75 years of age. The principal factors influencing the indication for screening colonoscopy are the risk of death from CRC, and the risks and benefits of screening itself. Cancer diagnosis alone does not benefit patients in terms of life expectancy; on the contrary, patients are at risk of complications from diagnostic and therapeutic colonoscopy. An individual approach to the completion of screening for each individual patient is in line with the emerging trend towards personalised medicine, but places far greater demands on the referring physician.
