Kardiogenní šok – komplexní terapeutický přístup
Kardiogenní šok patří mezi nejzávažnější a život bezprostředně ohrožující komplikace akutního infarktu myokardu.Přes rozvoj moderních diagnostických i terapeutických metod se incidence ani mortalita kardiogenního šoku zaposledních několik desetiletí zásadně nezlepšila. Časná a úspěšná reperfuzní strategie, především v podobě perkutánníkoronární intervence, je předpokladem úspěchu léčby akutního infarktu komplikovaného kardiogenním šokem.Do komplexní péče pak dále patří orgánová podpora: farmakologická i nefarmakologická stabilizace oběhu, včasnáventilační podpora s novými typy a režimy umělé plicní ventilace, podpora či náhrada renálních funkcí kontinuálnímihemoeliminačními metodami, metabolická podpora myokardu, rehabilitace a psychoterapeutická podpora včetněpsychofarmakologické intervence. Autoři uvádějí přehled současných názorů na incidenci, etiologii, diagnostikua terapii kardiogenního šoku.

Cardiogenic shock belongs to the most severe and immediately life-threatening complications of the acutemyocardialinfarction. Despite development of modern diagnostic and therapeutic methods the incidence and mortality ofcardiogenic shock has not significantly declined in the past decades. Early reperfusion strategy with percutaneousrevascularization has become a cornerstone of therapy. The complex approach to cardiogenic shock comprisespharmacological and mechanical hemodynamic support, ventilatory support utilizing new ventilator regimens,metabolic and renal support/replacement with continuous renal replacement therapies and psychological, eventuallypsychopharmacological support. All these measures enable prevention of the multiple organ failure syndromedevelopment and positively influence high mortality of patients suffering from cardiogenic shock.
