Summary

HIV encephalopathy is the infection of the CNS primary caused by HIV. The disease occurs mainly in the later stages of HIV infection in connection with profound immune suppression with CD4+ T-cells counts less than 200/μl. The clinical characteristics of this disorder are a cortical-type dementia with motor disturbance, in the terminal stages occurs spastic tetraplegia. In the case-report is described a course of illness of 32-year-old man with non-treated HIV-1 infection, in whom spastic tetraplegia, incontinence, vestibullar symptomatology and psychotic symptoms developed. CD4+ T-lymphocyte count was 60/μl and viral load RNA HIV in cerebrospinal fluid was 490 000 copy/ml. Starting of highly active antiretroviral therapy resulted in significant clinical improvement and decrease of the cerebrospinal fluid viral load.
