Summary

The authors present a case of 27years old man with sudden eruption of papulopustules and inflammatory nodules on erythematous and squamous base, without general symptoms. The dermal finding resembled acne fulminans, rosacea fulminans or demodicosis. The patient had no history of acne during pubescence, there were no comedones, no seborrhea, nor fever. No „flushing“, „blushing“,or ectasias. Demodex was repeatedly found by microscope in scales in great amount (>6D/cm2). The finding was confirmed by fluorescence.

Histology found fragments of the mite in follicles. Antibiotic and antifungal treatment had no effect. A complete remission was achieved after repeated systemic use of metronidazole and local application of a sulphur agent. The authors set the diagnosis of demodicidosis.
